DIO, the Joestar Family's century-old nemesis, intends to eliminate his rivals once and for all. To that end, he commands Noriaki Kakyoin, a fellow high school student and Stand user, to kill Jotaro in cold blood. Meanwhile, Holy falls ill from her own awakening Stand, leaving her with only 50 days to live.
Summary
In his shadowy lair, DIO mentions that Jonathan Joestar's corpse was a boon as it allowed him to survive at the bottom of the ocean. However, it now represents a weakness because the link between Joestars granted the members of the Joestar Family Stands at the same time as him. However, his confidante Enya tells her master she's taken dispositions to murder the Joestar.
In Japan, Joseph already notices the first symptoms of Holy's affliction. For his part, Jotaro is heading to school accompanied by his groupies. Jotaro sees a suspicious man in front of him and follows him into the forest. A fight ensues between Jotaro and Noriaki Kakyoin, wielding Hierophant Green. Jotaro is wounded but is angered at Kakyoin's callousness toward a nearby girl. Jotaro's Stand powers through the Emerald Splash and pummels Kakyoin, defeating him.
Afterward at the Kujo's home, the heroes examine Kakyoin and discover a flesh bud, through which DIO can control people. Avdol reminisces his terrifying encounter with DIO. Determined to save Kakyoin, Jotaro uses Star Platinum to extract the flesh bud and Avdol uses Magician's Red to destroy it. However, Holy then falls unconscious because of DIO's curse and the group learns that DIO is still in Egypt. To repay his debt, Kakyoin decides to accompany the group. The four men then prepare to go to Egypt, and Avdol christens Jotaro's Stand "Star Platinum". Unbeknownst to them, they are followed by someone and DIO is also feeling the group's impending arrival.

Manga/OVA Differences
- DIO commenting on Jonathan's body allowing him to survive is moved before Kakyoin's attack. The scene is combined with Enya's conversation with DIO, which is shown before Strength's introduction in the manga.
- A scene is added of Holy hiding early symptoms of her Stand fever from Joseph as the two discuss the incoming fax from the Speedwagon Foundation.
- Jotaro does not tell his fangirls to shut up. Instead, their back-and-forth ends when Jotaro notices Noriaki Kakyoin walking in front of him.
- Kakyoin's painting of Jotaro is removed entirely.
- Kakyoin no longer stops to offer Jotaro a bandage, instead walking further away into the forest. Instead, one of his fangirls gives him one later on.
- Jotaro and Kakyoin's battle takes place in the forest outside the school, rather than the school hospital.
- Hierophant Green possesses the schoolgirl that follows Jotaro, rather than the school nurse.
- Rather than using Emerald Splash immediately, Kakyoin attacks Jotaro with his Stand's tentacles.
- Jotaro deflects Kakyoin's second Emerald Splash with one arm rather than both. He then grabs Hierophant by the neck before counterattacking.
- Jotaro's return to the Kujo Mansion with an unconscious Kakyoin is omitted.
- Avdol's flashback to his encounter with DIO takes place on his way to his shop, rather than inside the shop itself.
- DIO's outfit in the flashback is changed to more closely resemble his outfit during the final battle.
- When Kakyoin's flesh bud digs into his skin, Jotaro sees flashbacks to DIO's past, including his fight with Jonathan at the Joestar Mansion and the final battle between the two on the boat.
- When DIO bursts through the roof of the mansion, his suit is still intact, though he is still shown without it as the two plummet through the mansion.
- The scene on the boat is reused from the opening. As such, Wang Chan's hands are again seen holding Dio's head after the fire has already begun.
- The flesh bud is incinerated by Magician's Red, rather than Joseph's Ripple.
- Holy's illness manifests immediately after Kakyoin is saved, unlike the original manga, where some time passes between the two events.
