Opening Animation
A lone helicopter flies over the Egyptian Desert. The desert sun fades into the sun above Mexico ages earlier, in the era of the Aztecs. The Aztec tribe is seen preparing for a ritual as the camera pans across their monument, and the engravings within. A topless maiden is shown being secured to a stone table atop the monument, her expression devoid of life.
The tribe begins to play on their drums and stomp rhythmically as their chief climbs the monument. As he approaches the table, the chief turns to reveal the Stone Mask he is wearing over his face. The chief then drags a scalpel across the maiden's body, stopping just above the woman's neck. Lifting the scalpel with both hands, the chief brings the sharp end down upon the maiden's throat. The chief then hoists the bloody scalpel in the air as the maiden's blood flows from the table's drains into bags prepared by the tribe.
In the modern day, a cruiser is seen using a crane to lift a coffin from the bottom of the sea. Inscribed on the front of the coffin is a single three-letter name: DIO. DIO is then seen within his lair, having killed several maidens in his lust for immortality. As he climbs a set of stairs, his shirtless body clearly exposes the star-shaped birthmark on his neck, stolen from Jonathan Joestar along with the rest of his body.
Back in the past, the chief pours the bags, now filled with blood, onto his mask. The mask absorbs the blood before suddenly sprouting several spines, which pierce the chief's head, and emitting a bright light. As the light dissipates, the chief, formerly a lean figure, rips his shirt to reveal his new youthful and muscular figure and cries out in victory.
At the Kujo Mansion, several doctors are seen attending to Holy Kujo, who has fallen into a coma. The cause is revealed to be Holy's Stand, which is shown being wrapped around the Stone Mask as it activates once again.
Altered Versions
From Episode 9 onward, the original music track, "Dio's Creation," is replaced by "Dio's Dervish." The pacing of the opening is also slightly adjusted to match the new track's faster tempo, and the sound effects present in the original version are removed. In addition, Episode 9 itself uses a shorter version of the opening that replaces many of the original shots with clips from the previous episode.

- The sacrificial maiden is depicted nude from the waist up, unlike in the original story, where she wears a plain dress.
